A child was observed watching You Tube videos on a tablet while laying down on a mat during the naptime in a pre-k classroom.

A caregiver supervising a group of children during the naptime did not know how many children were in the group. When asked for the number of children present in the room the caregiver stated there were 17 children, however there were only 16 children present.

Based on the information obtained during the investigation, it was determined that caregivers did not demonstrate good judgment when the caregivers allowed a child to remain asleep an hour and fifty-one minutes past the scheduled naptime resulting in the child being left unsupervised in the classroom during a practice fire drill evacuation.

Based on the information obtained during the investigation, it was determined that two caregivers left a child unsupervised in a classroom for 6 minutes during a practice fire drill evacuation.

During inspection, it was noted that two of the three outdoor play areas contained a significant amount of bird droppings, which hindered a classroom's ability to engage in outdoor activities. Additionally, plastic wrappers were found scattered on the ground, and there was broken play equipment in the area designated for children. A bird nest was also observed in the metal shade structure above the outdoor play area. All six classrooms, the hallway, and the kitchen were found to have dead insects in the light ceiling panels.

Training certificates were not available for review for three teachers.

Based on evidence gathered during the investigation two caregivers used prohibited methods of guidance at the operation. A caregiver forcibly redirected a child and pushed child while in care at the operation. Another caregiver grabbed a child with force as a form of guidance.

CPR/1st aid training was expired for one caregiver present during inspection. Training expired 1/25/2022. NOTE: Training has been scheduled for 2/4/2022.

4 children in two different classrooms were watching cartoons on a computer not related to planned activities. NOTE: Caregivers transitioned with the children to another activity.
